This session will be an introduction to Advanced Care Planning (ACP).

Topics covered will include:

• Basic understanding of what is Advanced Care Planning

• The benefits of Advanced Care Planning

• What Advanced Care Planning documents are available, including:

  • Medical Treatment Decision Maker

  • Advanced Care Directive

• How to complete the forms

• Understanding the legislation surrounding Advanced Care Planning

There will also be time allocated for any questions you may have in the session.
