Alfred Health Carer Services has been providing support to unpaid carers in Melbourne’s southern metropolitan region since 1996. Currently, we deliver three main carer support programs:

Victorian Support for Carers Program

Our friendly Admin team will usually be your first contact with our service.
The admin team is responsible for:
Answering calls to our direct number and responding to your requests for a callback through our website
Reviewing your registration and ensuring we have the correct consent information
Booking you in for a conversation with the Carer Assessment and Planning team
Supporting your service bookings and confirmations

The Carer Assessment and Planning team is our biggest team. They will speak with you about your current needs in relation to your caring role.
The team is responsible for:
Assessing your eligibility for our services
Talking with you to identify what would help you the most right now in your caring role
Working with you to develop a plan, identifying appropriate interventions and supports
Helping you to navigate the service sector and linking you to other services that may support you in your caring role and care relationship

The Wellbeing Team help to deliver a range of services to carers.
The team is responsible for:
Delivering Facilitated Coaching services for carers
Matching you to the right carer counsellor and seeking feedback about your experience
Ensuring you are matched to the right community peer support group and checking in to make sure that everything is going well with the group
Occasionally, providing additional short-term support to carers who may not be able to access information or supports without additional assistance

The Carer Engagement Team engage and collaborate with key partners and stakeholders to enhance social inclusion, participation, and advocacy for carers in the community.
The team is responsible for:
Promotions
Networking
Carer events and education
Carer engagement and outreach
Helping to establish new carer peer support groups and to provide links to already-established peer groups
Managing our eNews and website

The CHSP team delivers effective entry level services that promote independence, wellness and reablement whilst supporting care relationships for people aged 65 years and older (or 50 years and over for Aboriginal and Torres Strait Islander people).
The team is responsible for:
Managing your referral
Completing your program assessment
Developing your respite goal and plan and reviewing this with you
Supporting the delivery of flexible (in-home), residential day and cottage respite options

The management and operations teams ensure that our admin and clinical teams are well supported and resourced to adequately respond to your requests.
This team is responsible for implementing the services clinical governance and reporting protocols, working to ensure carers receive a high-quality service.
